This form takes about five minutes and gives us what we need to plan your week: who you are, how to reach you, what you eat, and anything that helps us set up the studio well. Please complete it within 10 days of receiving your confirmation. One form per participant. If you purchased a spot for someone else, list them below and we will send them their own form.
